What to Bring to a WIC Appointment

At the time of your WIC appointment there will be a brief encounter with a nurse. She will conduct a brief review of the foods you or your child eats, some medical history, and some current health indicators to determine a medical or nutritional need. If you or your child are found eligible for WIC benefits, the appointment will also include nutrition education and the assignment of a WIC food package. Prior to leaving the department you will be given WIC checks to use at the local grocery stores. 

What you should have ready for the WIC Appointment.
Proof of Identity: 
WIC requires a photo ID to validate identification. This may include:
  • Photo ID, drivers license, school ID
  • Birth certificate or passport
  • Military ID
  • Kansas Medical Card
Proof of Identity for your child:
  • Vaccination record
  • Birth certificate
  • Foster care or adoption papers
  • Kansas Medical Card
      (WIC cannot accept Social Security Cards as proof of identity)

Proof of Residency:
Envelope that was mailed to you

Completed Diet Questionaire:
The diet questionaire is specific for an adult or child and details lifestyle choices and eating habits.
Proof of Household Income (all people living in your house):

  • Paycheck stub from a current job
  • SRS benefits letter
  • Medicaid or Healthwave 19 card
  • Income tax return (self employed only)
