What to Bring to a WIC Appointment

Applicants will need to bring:

(June 1, 2021 we will resume in person appointments for WIC. You may choose to bring these items in or you may complete an intake questionnaire prior via online. WIC staff will continue to do appointment reminder calls throughout 2021.)

  1. Proof of ID for Caregiver/ Mothers
    •  Driver’s License or other Photo ID, work ID, Military ID, etc.
  2. Proof of ID for Infants/Children
    • Immunization record/pink card, Medicaid card, birth certificate or letter.
    • *SOCIAL SECURITY CARDS ARE NOT ACCEPTED AS PROOF OF ID UNLESS THAT IS THE ONLY PROOF AVAILABLE.
  3. Current proof of residency: lease agreement, utility bill, junk mail, etc.
  4. Proof of income: past 30 days worth of pay stubs, *if no pay stubs but you are working, please bring a letter from your employer with their letterhead, employer signature, how many hours per week and rate per hour earned.* SRS award letter, child support statements, Tax return, etc.
    • If you were impacted by COVID-19, you may bring the 1040 tax return form from the previous year, unemployment checks/direct deposit bank statements, the last 30 days of pay stubs, or recent additional sources of income.
  5. Diet Questionnaires (click on the appropriate fillable forms needed for the certification appointment- once submitted it will email WIC staff.)
