Applying For WIC:

The Ellis County WIC Program serves residents of Ellis County who are:

  • Pregnant
  • Breastfeeding: up to the baby’s first birthday
  • Postpartum: up to six months after infant’s birth
  • Children under the age of 5

First Time Applicants:

New applicants must come in to the Ellis County Health Department with proof of residency & ID of each applicant:

Caregiver/Mother: Photo ID (driver’s license, work ID, etc.)

Infant/Child: Immunization record, birth certificate or letter, or Medicaid card

Proof of Residency: Utility bill, lease agreement, a piece of junk mail, etc.

To RE-APPLY or Transfer-In:

If you've been on WIC before and need to reactivate due to a new child/pregnancy or transfer your existing account from another county, please bring in the following:

Caregiver/Mother: Photo ID (driver’s license, work ID, etc.)

Infant/Child: Immunization record, birth certificate or letter, or Medicaid card

Proof of Residency: Utility bill, lease agreement, a piece of junk mail, etc.
